NEW PATIENT INFORMATION

We accept new patients into our practice only upon referral from another medical provider.  This is required by our physicians and is not a insurance-related concern.  Please ensure that your doctor or their office has contacted us by sending a Request for Consultation and forwarding your pertinent medical records to our office.  Our doctors review all new consult requests along with your records prior our staff scheduling an appointment in our office.  Please contact your referring physician to ensure that the referral/consultation was initiated on your behalf. 

 

Our doctors set aside a significant amount of time to review information sent to them by other doctors, labs and hospitals. Please arrive 15 minutes early for your appointment.  Your initial visit could last several hours so plan accordingly. We require you give the office  24 hours notice in the event you need to cancel or reschedule the appointment.  Failure to keep a new patient appointment may result in the inability to reschedule this appointment.  There are patients who are on a waiting list for appointments and we would like to accommodate them if you decide not to keep your appointment.

   

Due to the nature of our practice we respectfully ask you not to bring small children with you for your appointment. If you need an adult with you, please limit it to one person.

 

Summary of what to bring to your FIRST appointment: 

  • Picture ID (Driver's license, Passport, Student ID)
  • ALL Insurance Cards (Copays are expected at time of service)
  • A list of your medications - both prescription and over-the-counter medications.  It is important to know the dose and frequency that you take these medications. We encourage you to bring the bottles if you are unsure.
